When most people hear the word anemia, they immediately think of iron deficiency. While iron deficiency anemia is common, anemia is actually a broad term that describes problems involving red blood cells or hemoglobin that reduce the body’s ability to transport oxygen efficiently.
Understanding the different types of anemia and what your lab work may be showing can help paint a more complete picture of what’s happening in the body.
What Is Anemia?
Anemia is not a single condition—it’s a category of disorders affecting red blood cells or hemoglobin. These changes can reduce oxygen delivery to tissues, often contributing to symptoms such as fatigue, decreased exercise tolerance, and low energy.
Healthcare providers often classify anemia based on two primary characteristics:
- The size of the red blood cell (measured by MCV)
- The amount of hemoglobin inside the red blood cell (measured by MCHC)
The Three Main Types of Anemia
Microcytic Anemia
Microcytic anemia is characterized by red blood cells that are smaller than normal. Iron deficiency anemia is the most common example, but other conditions such as sideroblastic anemia and thalassemia also fall into this category. Vitamin B6 deficiency can also affect normal hemoglobin production.
Normocytic Anemia
In normocytic anemia, red blood cells are normal in size, but there are still problems with oxygen transport. Blood loss, autoimmune conditions, and hemolytic anemia are examples that may fall into this category.
Macrocytic Anemia
Macrocytic anemia occurs when red blood cells are larger than normal. This pattern is commonly associated with deficiencies in vitamin B12 and folate (vitamin B9).
Iron Deficiency Isn’t Always the Whole Story
Iron deficiency anemia is often associated with blood loss, such as menstruation or gastrointestinal bleeding. However, it’s important to understand that iron deficiency may have multiple contributing factors.
Additional factors discussed include:
- Gut, oral, fungal, bacterial, or parasitic infections
- Vitamin B deficiencies, including B6, B9, and B12
- Increased nutrient demands related to methylation
- Metabolic and blood sugar dysregulation
- Autoimmune or hemolytic processes
Lab Markers That Can Help Evaluate Anemia
A complete blood count (CBC) provides several important markers that help classify anemia and identify patterns.
Common markers include:
- Hemoglobin
- Hematocrit
- Red blood cell count (RBC)
- Mean Corpuscular Volume (MCV)
- Mean Corpuscular Hemoglobin Concentration (MCHC)
- Red Cell Distribution Width (RDW)
Iron studies can provide additional information, including:
- Serum iron
- Ferritin
- Total iron binding capacity (TIBC)
Looking at these markers together can offer more context than evaluating a single lab value alone.
Looking Beyond Iron
Anemia can result from multiple underlying factors, which is why identifying the specific type is important. Depending on the individual situation, evaluation may include nutritional status, blood sugar regulation, iron studies, B vitamin levels, and other laboratory markers.
Rather than viewing anemia as simply an iron deficiency, understanding the different classifications and associated lab markers can provide a more complete picture of overall health and guide more individualized care.
